Photo and Video Release Agreement

I , do hereby grant KUTIO LLC irrevocable permission to use my photograph and/or video tape and/or written testimony for use in various publications and media (including but not limited to cassettes, videos, radio, T.V., internet, brochures, etc.). All rights pertaining to all publications and media uses made hereunder shall remain vested in KUTIO LLC.

I grant KUTIO LLC permission to use my image and voice, and give all rights to use my image and voice in any and all commercial or other interest in which KUTIO LLC is concerned.

Signer warrants that he/she is authorized and of legal age to fully enter into this agreement, and grants permission here within.
